Supervision Agreement

1 In accordance with the guidelines for supervision, set out in the APHP documentation, this is the formal agreement between the therapist and supervisor as named above.

2 Agreed to undertake a minimum of 5 - 10 sessions (depending on client numbers) of supervision over a period of one year. This will normally be via Zoom but telephone contact will be acceptable if required.

3 The fee will be £65 per hour or £85 for 90mins. Short telephone calls, voice messages and emails exchanged in between sessions are free of charge, but for telephone work of 30 minutes or more, an accumulative fee will be charged.

4 The principle focus of supervision will be work in progress with specific clients but discussion of protocols, new techniques, marketing and ongoing training will also be appropriate.

5 The general structure of the meetings will be

· Follow up from previous meeting, reviewing progress

· Current client issues including reflective practice

· Development priorities (CPD)

· Any other issues

6 Both supervisor and supervisee to keep a record of meetings, the time and date as well as content. In accordance with the APHP requirements and if requested, the supervisee will submit a supervision record, signed by the supervisor, to APHP.

7 The supervisee to keep a notebook regarding their personal development and practice and other notes as deemed appropriate.
